Here in Tulsa we have the only surviving intact Spartan NP-1. It hasn't flown since coming to Tulsa a few years ago, though the engine has been run and it is rigged to fly... kinda depends on your definition of "flying." But it is the only one around. The other that shows up on the FAA registry is a pile of parts.

I believe Rich Sudgen's FJ-4B is the only Fury still airworthy.

Our C-123K has been the only one actively flying for the last couple of years now.

Just speaking to Spartan Aircraft types...

In Tulsa we have the one and only:

Spartan Model 12- it flew in last year
Spartan NP-1- last flown 4-5 years ago, engine run since
Spartan C3-225- one is at the EAA museum, but it hasn't flown in years

Ed Wegner just sold his Spartan C3-165, which is one of two survivors and the only one flying

At WAAAM in Hood River, Oregon, the only flying Spartan C2-60 is located. Flies on a regular basis.

How's that?

Historic Flight Foundation's B-25D "Grumpy" is the oldest flying Mitchell around and only "D"

Of corse which has already been mentioned once, I believe, is FHC's Fw-190A5. Only flying original airframe and only (for now) working BMW.
I would imagine historical significance would allow their IL-2 to be a decent exception to the engine rule.

They used to be the only ones flying an I-16, but that seems to have gone on the back burner.
