Too long of a crew day. I flew some 24 hour missions in the first Gulf war, my butt was dragging by the time we landed at O'dark thirty. It was easy to make mistakes. I called a go around once, as an FE, because the two pilots were dead tired.

That was a MC-130H Combat Talon 2. Trying to remember the specifics of that one. There was talk that the hazard wasn't listed, or that crew missed it. All I know for sure is, they briefed us up yazoo after that.

I thought that one was a forward base in Afghanistan. Like Mike, I heard it was a runway project that wasn't NOTAMed. The plane came in at night, and the field was uncontrolled, with the result in the pics. I believe there were some broken bones and other injuries, but fortunately everyone survived.

Iraq. Construction being done without NOTAM. Although the current generation of NVGs is wonderful, it's completely understandable how this pothole, especially because its edges were not ragged, could have been unseen or mistaken for an asphalt section. Thank goodness no one was killed - beyond that, a Talon is a major national resource, what a shame to lose one.
