This just in from the Naval Historical Center, Washington Navy Yard:
The ship is the USS Hinsdale:

They said, "Morison lists the following as having been damaged off Okinawa (aside from those sunk): Alpine (1 April), Hinsdale (1 April), Henrico (2 April), Achernar (2 April), Pinkney (28 April), and Sandoval (28 May). His descriptions of the damage received indicates that it isn't Henrico (hit in the superstructure), but I can't tell about the others. What struck me while going through the list to break out the transport types is how successful the efforts were to keep the kamikazes away from the offloading areas, if only by putting other ships along the line of approach (in effect as bait targets). Lots and lots of warships were hit for every transport or freighter."
Cheers,
