The US Army Aviation accident investigation Board and possibly records should be on file at the Aviation Safety Board at Ft. Rucker, Alabama. Given the RPMs of the tail rotor drive shaft, a ball hammer, anything is possible. Then again, it could have been a hanger bearing failure which caused the failure. Was there in FACT a ball hammer found in the tunnel cover?
