Cause of Aluminum Overcast's mishap

I never heard, but what caused Aluminum Overcast to have the gear failure at Van Nuys a few years ago?

Just a guess, but since there was no guard on the gear switch, and it is right next to the flap switch, well, you get the picture.....................
