Little Giant kit for 12D40 prop

We have a little giant seal kit installed on our T6 12D40 prop. We installed it years ago and have lost the STC installation leaflet that came with it. Has anybody got a copy of the original STC that they could Email to me? Thanks.

I will see if I have a copy. Butch Maxwell at Maxwell Propeller might have it as well.
