Just to follow up on this point, apparently the weight and balance paperwork is still called "Form F".

Apparently, at least now, there is a whole series of letter forms:

The above all come from the DoD Forms Management Program page on the Executive Services Directorate website. I also found the technical manual for using the forms.

It's worth noting that the wartime "Handbook of Weight and Balance Data", in addition to Form F, has a "Cargo Loading Supplement". While I haven't found an online copy of the wartime version yet, I did find a version of Form F from 1950.

Finally, while they're not USAAF forms, MOTAT has posted some high quality scans of RAF Form 700 - Aircraft Servicing Form: 1, 2, 3, 4

I took the opportunity to scan a booklet at the museum today that has copies of the forms mentioned in the previous post. It is the Handbook of Weight and Balance Data, AN 01-1-40, dated May 15, 1943. It is a series of blank forms that is apparently intended to accompany a specific airframe. I only scanned the forms themselves, but there are also a bunch more pages of instructions.

Wow! What a thread. Thanks for posting all of these. One of the guys in our reenacting group is our clerk. He's been collecting all the forms to properly fill out his field desk, but he likes to get and recreate the USAAF ones to fit our camp as well. His handles on some other forums is AirCorpsClerk :)

I'll be sure to share this thread with him.

Thanks! I looked up some of his threads (1, 2), and man, he is dedicated! I hope it is able to help him!

I must admit, I initially started this thread in large part just to satisfy my own pedantic fascination with organization and record keeping, but between you and Mr. Lynn, I am pleasantly surprised that anyone else finds this interesting or useful.

Also, while searching "AirCorpsClerk" I happened across an interesting news article about an actual USAAF clerk. He would be the guy to ask!

Just the other day I found a very similar effort to this thread, except it is for medical, and not AAF, forms. One version is is from a website called WW2 US Medical Research Centre. However, it may be a copy of another one from a website called U.S. Army Data Depot.

I came across these pages in a class book (Group C; Squadrons 13, 14, 15, 16, 17, 18; Officer Candidate School; First District Army Air Forces Technical Training Command; Miami Beach, Florida) that was recently donated to the museum. While they aren't forms themselves, the pictures do show the environment the forms were used in (plus some unique pictures of olive drab, two tone camouflaged O-47s as a bonus).

Something I didn't notice until recently, (although I should have when I posted a list a little while ago) is that AC and AAF Forms 24 are different. The former is titled "Flight Performance Record" while the latter is the "PIF Table of Contents".[1] The same reference that made me realize the above also states AC Form 26A is actually a book made up of AC Form 21A.[2]

I came across a catalog on AirCorps Library today called the Index of Army-Navy Aeronautical Equipment - Miscellaneous, T.O. No. 03-1-46, dated 10 July 1945. It has five pages that have either forms or their associated equipment. The applicable equipment is:

It's worth noting that the holders have a similar function to the bag from Flak-Bait and the bombardier's instrument calibration cards are similar to the tangent scales for the automatic bombing computer associated with the Norden bombsight.

Just under a year ago, someone donated a collection to our museum that belonged to a man named Harold A. Baumgartner which included a form called a "Classification Folder". A literal folder with a form printed on it, it had within it forms in the following post. While it may not technically be an AAF form, I feel it still fits in the general theme of this thread:

The forms below came from the collection at the museum that belonged to a man named Lee R. Dukes. Note that while the first one not technically an AAF form, it still seemed relevant enough to include like the Personnel Folder above:
